First Visit Preparation

  • How to prepare for your first visit

  • Your initial consultation is where our team will gather information about your history, so we can offer recommendations personalized to your situation. To help us provide the best possible care, there are some important steps for you to follow beforehand. 

     

    Our Patient Services team will send a set of medical history forms that you will need to complete and return prior to your scheduled consultation.

     

    Your initial consultation is an opportunity for us to get to know each other. If you are an Ontario resident, your consultation is completely covered by the Ontario Health Insurance Plan (OHIP). 

     

    During your first visit, you will meet with one of our Fertility Specialists to establish a thorough understanding of your reasons for seeking fertility assistance. There are no tests during your first visit, and you do not need to have had tests prior to your first appointment. However, if your referring doctor had you undergo tests, those results will be sent along with your referral.

     

    Make sure you bring your health card and any existing medical test results.

    Once the Fertility Specialist has a good understanding of your situation, they will recommend certain assessments. Most assessments incur no additional costs, but some will. If additional costs are required, your doctor will review them with you before moving forward. Once you determine which assessments you need, you will return to undergo them at a later appointment.

    After you have completed your assessments, you will have an appointment for review. During this third appointment, you and your doctor will go over your assessment results. From there, you will discuss their recommended fertility care plan, as well as any associated costs.
